Schedule A 
1-5 Employees-$250
6-20 Employees-$250 plus $7 per employee
21-50 Employees-$355 plus $5 per employee in excess of 20 5 
50 + Employees-$500 plus $3 per employee in excess of 50

1. Multiple Locations (same owner and business)
Follow Schedule A for total number of employees for all Shoals area locations, plus $50 for each additional location in the Shoals to receive mail and benefits

Follow Schedule A for total number of employees at location of largest employment base for membership investment. Each additional location may be added for $150

2. Small Business Blanket Investment
(same owner, separate business)
The rate only applies when the owner of the first business owns 100% of the additional businesses. Follow Schedule A for total number of employees at business with largest employment base. Each additional business may be added for $150. 

3. Professional
Follow Schedule A for employees of professional companies and first professional. Each additional professional member is $150 (i.e. Accountants, Architects, Attorneys, Chiropractors, Consultants, Dentists, Doctors, Engineers, Opticians, Optometrists, Veterinarians, Real Estate Agents, Insurance Agents, Stock Brokers, etc) 

4. Financial Institutions
$250 plus $20 per million in deposits 

5. Hotels/Motels
Convention properties-ballroom seating-$250 plus $3 per room Full service with restaurant-$250 plus $2 per room Lodging only-$250 plus $1 per room 

6. Apartments and Campgrounds
$250 plus $1 per unit/size 

7. Not for Profit Clubs/Organizations and Churches - $225 

8. Individual
(Not affiliated with any business as an owner, manager, employee or
contract agreement, have no employees and represent only themselves)
$150 

9. Essential Public Entities
(These are public organizations that are critical to the well being of the community) Negotiated Rate (i.e. Utilities, Health Care Facilities, Local Government, etc.)
